eServices: eForm 6 To better meet your needs and provide you with more accessible service, the WSIB is expanding its eServices. We are pleased to offer you the new eForm6 which is an online alternative for submitting your Form 6 information. Electronic submission is an easier and more efficient way for you to submit your Form 6 information to us. However, our procedures in processing your claim remain unchanged. Therefore we cannot promise you that submitting your form electronically means it will get processed faster, or that entitlement decisions will be made any sooner. The adjudicator will review your eForm 6 within a few days. You may wish to call your adjudicator after a couple of days if you have not heard back. To use eForm 6, you must know your WSIB claim number. To log into the system, enter your current claim number and your date of birth. Log in now.
