    Payments for No Record Form 8s

    The WSIB is now paying for Health Professional’s Reports (Form 8) (486k, pdf) in cases where a worker cannot be assigned a claim under the Workplace Safety and Insurance Act (Act). While unusual, this situation can occur when an employer is either not registered or not covered under the Act. The effective date was January 1, 2007, on a go-forward basis.
      We recognize that health professionals complete Forms 8s in good faith when a patient identifies that they have suffered a workplace injury/illness. To date, the WSIB has not been able to pay for these forms when submitted. We have now developed a process to allow eligible health professionals to be paid for the completion of the Form 8 even when coverage is not indicated.

      Health care professionals’ remittance statements now contain a new explanation code that will identify these cases. When a claim number cannot be assigned to the patient, the note “Worker Not Assigned a WSIB Claim – One Time Payment” will appear. Payment will only be made for the completion of the Form 8 and does not extend to continuing care or treatment.

      The WSIB reminds health care professionals to:
      • complete all the patient identifiers on the Form 8 for prompt payment
      • provide their nine digit provider number, including name and address
      • sign the form.
